Assume primary responsibility for pediatric patient/family using the nursing process to assess, plan, implement and evaluate patient/family needs, goals and interventions.
As applicable, apply specific criteria for admission and re-certification to hospice care or hospice waiver care to establish appropriate levels of care and patient’s eligibility for children.
- Initiate communication with attending physicians, other hospice staff members, facility staff members as applicable, and other agencies as needed to coordinate optimal care and use of resources for the patient/family.
- Maintain regular communication with the attending physician concerning patient/family care needs and response to interventions.
- Implement the individualized plan of care and recommend revisions to the plan as necessary. Determine scope and frequency of services needed based on acuity and patient/family needs.

Requirements & Skills 1

One year experience as a professional nurse in the past three years or; Bachelor’s Degree in Nursing from an accredited School of Nursing.
One year pediatric nursing experience required.
Experience in acute care/ home health and/or hospice setting preferred.
Current license as a registered professional nurse in the State of California.
- Public Health Nursing certification highly desirable.
- HPNA certification preferred.
- Current Driver’s License and automobile insurance that meets State requirements, if driving a car.
