Hospitalist Job Description

Job summary 1

The Hospitalist, in collaboration and consultation with physicians, staff RNs, and other health care professionals, provides holistic, compassionate care to inpatients and their families. The Hospitalist will manage the care of customers hospitalized with complex acute health problems, such as trauma, coronary artery disease, cancer, diabetes, and heart failure. T he Hospitalist will be a member of the hospital's interdisciplinary team and facilitate a continuum of care for customers from admission through the discharge process, including facilitating the customer's transfer of care back to the primary provider and consulting and making referrals to physicians and other members of the health care team as appropriate. The Hospitalist will also help organize the workload and facilitate provider groups to streamline the care of customers on the service.
