Under the guidance of the Care Connections Manager, the Care Coordinator serves as a vital member of an Interdisciplinary Team for Health Home Department and plays an active role in designing an integrated care plan for children and their families. As the first point of contact for clients, Care Coordinators are primarily responsible for completing assessments, making and tracking referrals, following up on referrals, and coordinating with internal team members and other professionals.
