As a Patient Navigator, you will serve as the first point of contact for 9-1-1 patients that are connected to Hospital. Similar to First Responders and 9-1-1 dispatchers, you will utilize your training and experience to provide a seamless “9-1-1-to-telehealth” experience for Hospital’s patients. You will be responsible for greeting callers, executing patient intake, and completing mental health and substance abuse evaluations before matching the patient with the appropriate care resources.

This position is a high-touch Patient Services role centered on delivering personalized support to patients within the Rare Disease patient journey. The Patient Navigator (PN) is a key patient-facing role responsible for executing on case management activities spanning from patient welcome and enrollment through onboarding, serving as the primary point of patient contact. The PN will provide onboarding support from enrollment into Alexion’s OneSource support program through first product administration, including help in navigating insurance approval and site of care selection. The PN will also manage any financial assistance program enrollments or referrals for patients. Additionally, the PN will also provide education on patient services, disease states, and Alexion products, during patient interactions.

The primary function of the Patient Navigator position is to connect/reconnect patients admitted to Hospital with their assigned primary care medical home upon discharge. The goal of the program is to improve patient outcomes, reduce unnecessary emergency room visits and preventable hospitalizations at Hospital and to promote the utilization of a primary care medical home. The Patient Navigator will work with patients, primary care providers and staff, providers and staff to maximize patient use of appropriate and established health services; assist and guide patients in receiving post hospital follow up visits; educate patients on the appropriate utilization of care, appointments and treatments services; ensure that patients are able to access and keep medical appointments, and follow-up with patients as needed. This position will require flexibility with respect to responsibilities as the program evolves over time. Must be comfortable working in a hospital/emergency department setting.

In this position, you will be responsible for providing support in coordination of care activities and assisting patients to navigate through all the steps in obtaining the necessary and appropriate care. The Patient Navigator is responsible for helping to ensure a seamless transition from inpatient to post discharge follow-up in the outpatient clinic.
