Social Worker

Summary

The Social Worker will provide comprehensive interventions to manage the highest risk and highest utilizers of services within the Hospital and Emergency Departments. The Social Worker will work closely with inpatient and Emergency Department Interdisciplinary teams, community medical providers, mental/behavioral health providers, homeless services, and substance use providers, to address any biopsychosocial determinants related to patient care.

Responsibilities

  • Conducts comprehensive initial assessment of patients to evaluate social, emotional and physical needs by applying social work theories and techniques; interviews and evaluates patients and family members.
  • Identifies factors which may interfere with obtaining maximum benefits from treatment and implements appropriate plans to address.
  • Collaborates with interdisciplinary health care teams and providers to treat and plan for the social and medical needs of patients. Devises, develops and implements treatment plans. Provide clinical Social Work interventions, Care Coordination, access to care and resources to provide proper utilization of services to a panel of patients.
  • Prepares and maintains records and makes written and oral reports to document case activity concerning patient care, counseling and therapeutic activities in accordance with department, hospital, medical center and/or health system policies. Prepares reports and other documents concerning patients. Participates in monitoring and evaluating the quality and appropriateness of patient care.
  • Ability to document, review, and interpret data necessary to provide services.
  • Provides supportive counseling, grief counseling, crisis intervention and other appropriate therapeutic services to assist patients and families in coping with symptoms and problems such as, adjustment to illness, accepting medical recommendations, loss of function, and adjustment to social circumstances.
  • Works closely with community providers and participates in community provider meetings.
  • Provides psycho-education to patients and providers, assists with health literacy and adherence, and addresses Social Determinants of Health.

Requirements

  • Master's degree in Social Work (MSW) from an accredited college or university
  • This position requires an active SW license
